According to the statements, the woman alleged that her identity document was issued from a location she had not used, a discrepancy she reportedly discovered in 2001 when attempting to register her newborn child. Officials said records discussed during the session showed an infant registration under the name Chidimma Vanessa Onwe Adetshina.

The discussion also touched on procedural steps taken by the Department of Home Affairs, including a Promotion of Administrative Justice Act notice issued to Adetshina's mother on 7 August 2024 to allow for formal representations.
